I have to assume that this item is a one-time reward for the first completion of The Shiftling's Wise Judgment if you choose Katsuie, so as to reward you with the dialogue changes to the later mission(s). Although I do know that when I came back to that mission and completed it from both perspectives (I had chosen not to side with either and completed the mission that way the first time and never tried the mission again until later), I was able to do other missions, so maybe you get it anyway. idk why Katsuie would give you an item and not the shiftling, but... well, it's possible.
